Error when try to install FormaLMS

I am using ubuntu 22.04
install apache2
install php7.4
install mysql

done extract and copy formalms 3.3.22
but I'm getting error when do
http://192.168.x.x/formalms/install

This page isn’t working192.168.x.x is currently unable to handle this request.
HTTP ERROR 500
