It was a combination of things. Checking the error log messages. Making sure all file permissions were correctly set, checking php was the correct version, making sure correct php extensions were enabled then checking the apache server had the correct settings enabled. Then finally check the scorm file itself. Somewhere in all of this was the solution but it was a combination of things and not one in isolation. I suggest treating it as a process of elimination and if you do not have the experience to check it all, ask for help from your web host / support / web developer.
