Unsubscribe course issue

Hello,

I just installed forma for evaluation recently. I observed an issue on course unsubscription email is not sent to user (althought "User unsubscribed from a course" is turned on at event manager). Also if approval is enabled and user clicks on course unsubscription, the admin user can only see the course is in progress for that user and not able to approve his/her unsubscription request.

Is there a bug or my set up issue?
